If you are not connected to our Physician Portal and would like to review your patients’ reports and images online, please complete this form and we will contact you within one business day to set-up your account and answer any questions you may have.

Please note: This portal is for physicians and medical offices only.

ACCURO

Follow these E-Form Installation Instructions:

  1. Open Accuro and select the Accuro “Bullseye” (red icon at bottom-left of the screen).
  2. Click “Tools” then “Form Editor”. A new box will pop-up.
  3. Click “Tools” again then “Publish/Download”.
  4. Change category from “Standard Forms to “Requisition Forms”.
  5. Search for your applicable requisition form e.g. PDXN Centre – [Location]. The Publisher should be “PDXN”.
  6. Click “Preview” as needed then click “Download” to complete.

OSCAR

Step 2: Follow these E-Form installation instructions:

  1. Open Oscar and click the “Administration” tab (user login must have administrative access).
  2. From the side menu, click the “Forms/eForms” tab then “Upload an Image” from the sub-tab.
  3. Click “Choose File” and locate both JPG and the eform.properties.js files saved on the desktop from the previous step.
  4. Upload all 3 files one by one selecting the file then clicking the green “Upload” button to complete.
  5. Next click on “Manage eForms” then “Upload” located beneath the “eForms Library” title.
  6. Before choosing the file to upload, title the form, ask the physician’s office if they prefer to see our company name first or “Radiology PDXN Centre”.
  7. Click the “Choose File” locate the HTML file from the desktop and click “Upload” to complete.
  8. Once completed, have the reception staff quickly test out the new form using a test patient.

If you have a faxing function set-up, there is a second step to add PDXN Centre as a provider. This will allow you to select PDXN Centre from the drop-down menu at the bottom of the e-form and fax the form directly:

  1. Click on “Consultation” at the top of the Oscar homepage.
  2. Depending on your version of Oscar, either “Add Provider” or “Add Specialists” should appear. Here you can add PDXN Centre contact information (address, phone number, fax number).
  3. Enter “Centre” as the last name and “PDXN” as the first name and click “Save”.
  4. Click “Edit Specialists Listing” and then click “Radiology”. In the list of specialists, select the check-box for “PDXN” and then click “Save or Update these Services Specialists”.
  5. PDXN is now appropriately associated under radiology in your system.

PS SUITE (TELUS PRACTICE SOLUTION)

Step 2: Follow these E-Form installation instructions:

  1. Open PS Suite.
  2. Open a test patient's chart.
  3. Go to "Settings" (towards top-left of the screen).
  4. Click "Edit Custom Forms" and a new page will pop-up.
  5. Click "File".
  6. Click "Import Forms".
  7. Select the applicable requisition from your downloaded zip file.
  8. Click "Download".
  9. To test the requisition form. open a test patient's chart again and go to "Data".
  10. Select "New Custom Form" and this will open the requisition into the patient's chart.
  11. To add the physician's signature. double-click on the signature box (this will only work if you are Logged in under the physician's account).
